US child passports generally need both parents or a notarized consent. Bring the relationship evidence the form lists. Appointment no-shows waste weeks. If one parent cannot attend, read the current consent form, not a forum post.
Adding a shorter version: do the checklist in the accepted answer, then wait 15–30 minutes before retrying. Most “it still failed” reports I see are immediate retries.
