US child passports generally need both parents or a notarized consent. Bring the relationship evidence the form lists. Appointment no-shows waste weeks. If one parent cannot attend, read the current consent form, not a forum post.
ESTA died the day the new passport number hit. Re-applied before the flight, not at the airport.
